You don't need to put every single date and offence where there were multiple offences at around the same time, i.e. suppose you were arrested for 62 shop burglaries in 1991, and convicted of them all at the same sitting at whatever crown Court - you need only say that you were convicted of Shop Burglary in 1991. You don't need to frighten off potential employers by sending them 16 closely typed pages of say, 139 offences, when most of them were the same type of offence committed at around the same time.
As with the CV, keep it as short and simple as you can without telling lies. If they want "Full" disclosure, you can do that once you have a foot in the door (after you've got to the interview stage, where you've got the chance to tell it your own way before they get the heavy full list of offences.

In respect of my application to work for the company you represent, I am duty bound to inform you that I have a criminal record, for offences that happened from 19?? – to – 200?, (i.e. when I was ?? years old, up until I was ?? years old).
Those offences ranged from ( for example ) :--
taking vehicles without the owners consent (!970 - 1980)
burglary of chemist shops during the years (1972 – 1977) when I was abusing various drugs,
armed robbery (1980)
wounding a police officer with intent to resist arrest during the course of robbery (1980).
I haven’t been involved in drug taking, or criminal offending since that time, nor have I the intention to do so in the future. But because of the sentence / parole issue, it would be a new criminal offence for me to withhold that information from you, hence my disclosure herein.
I would hope that you could take the view that my criminal past was a phase I went through in my youth, and that it should not be an automatic bar to my being considered for the post for which I am applying now.
If you have any concerns arising from this issue, I will be happy to discuss the matter at interview.
