You will need a CV for sending off to some of the companies you'll be applying to for work. There are no real rules for CV's, but everyone you ask will have a different opinion to voice on the subject. Most employers just want a snap-shot picture / description of you and your abilities / achievements, so they can work out whether you'll fit in with their workforce and needs. So don't try to cram in so much that they can't see the woods for all the trees in the way. Here is a link to Learning For Life's CV help pages which has a 3 examples of different types of CV that you can change / copy for your own use, and a basic tutorial in writing your own CV. If that doesn't go far enough for you, this link goes into more depth to help you write a better CV.
Below is the format that I personally favour. The basic rules are - keep it simple and never do more than 2 pages of A4. As a word of advice, I would add the Disclosure Letter to the back of this, which will save you any arguments / frustration in the long run.
