Please fill in the details of the person who will be financially responsible for this booking in the first section of the booking form. All correspondance regarding the booking will be sent/emailed to this person.

If booking for more than 1 person, fill in details of the other people in the following three sections. If booking for more than four people, continue on another copy.

In the 'Fees' section, fill the left hand with the number of each category of delegate being booked and fill in the total cost for that category in the right area.

Add the amounts in the right hand fields up and put the total in 'Total Fees', then fill in the remainder of the fields as indicated.

Remember to read the rates and accommodation pages as well as these instructions before filling in the form.

You are welcome to send the full fee with the booking.

Please send your final payment no later than 15th April 2008, otherwise we cannot be certain that you are definitely attending Conference.
