| Description |
Crisis Resolution is a new
multi-disciplinary service to provide rapid screening and assessment for
people referred to specialist mental health services. People are
assessed in their own homes and their needs identified. The service
provides intensive home support and treatment with facilities for crisis
management and respite. The service is focussed on the individual,
providing a person centred, respectful service with the collaboration of
other people important to the individual. Support options emphasise
minimum disruption to the person's life. |
