You can book a place by
downloading the application form from the link below and sending it with
the appropriate cheque made payable to the 'United Lincolnshire
Hospitals NHS Trust' and send to DOW, Maxillofacial Unit Clinic 9,
County Hospital, Lincoln. LN2 5QY.
Last Booking.
By Friday January 22nd 2010.
Cancellation Policy:
If
you find you are unable to come please let us know as soon as possible
in writing. Refunds will be given on receipt of written request if
received by Friday Jan.22nd. (a telephone call will not do). You can e-mail us
at contact@sorejaw.co.uk. Following Jan. 22nd our costs will
be fixed so refunds will not be made. However if you do find you cannot
come then let us know as if there is a late application they can be
balanced and a refund given. We cannot give refunds for no shows even if
illness is claimed as the reason. There is an administration charge of
£20 for cancellation where a refund is paid.
Transfer of
bookings.
Bookings are not transferable from one person to another.

£185 plus £20 if you want to bring a partner to the dinner

Credit cards.
Principally for
the benefit of those booking from overseas or at the last minute we can accept
MasterCard &
Visa credit cards. We prefer cheque payment as this avoids us paying
credit card fees. If you would
like to pay by card please put your card details on the booking form and
fax it to us on: 0044-1522-573455.
Confirmation.
When we have received your booking request we will send confirmation and
a receipt for the course fee by e mail. |
